We collect the information needed to prepare for your visit and review insurance participation. Your health plan determines coverage and patient responsibility.
03 Meet with Dr. Dere
Your initial visit is a comprehensive cardiometabolic evaluation focused on your medical history, medications, prior testing, weight trajectory, risk factors, and goals.
04 Understand what matters most
Dr. Dere explains the key findings and helps you identify the issues that deserve attention now.
05 Build your medical plan
Your plan may include medication, nutrition, physical activity, sleep, home monitoring, additional testing, or referral when clinically appropriate.
06 Choose your level of support
Continue with insurance-based medical follow-up according to clinical need. If you want monthly physician-led lifestyle coaching and more structured support, you may add WellifyCare Guided Care.
07 Reassess and adjust
Cardiometabolic care changes as your health, treatment response, and priorities change. We follow relevant information and adjust the plan when appropriate.
